ADDITIONAL INFORMATION REGARDING REQUIRED SCHOOL FORMS
Each family's information must be completed/updated in your My BackPack profile. Your family's listing in the School Directory will be printed from the information we have in the system as of July 25, 2014. Please login at https://tampaprep.seniormbp.com/SeniorApps and verify that your information is up to date.
CONSENT AND AUTHORIZATION FORM
The School's Consent and Authorization Form must be read and signed by you and your student. The Consent and Authorization form must be returned to the School by July 25, 2014.
All NEW students must submit a copy of their birth certificate. This is to maintain our accreditation with the Florida Council of Independent Schools (FCIS) and Florida High School Athletic Association (FHSAA).
Every student is required to have a yearly physical examination. Each physical is good for one calendar year. So that our records will be consistent, all students, including Middle School students, should use the FHSAA physical examination form (EL-2 above). Page one must be completed and signed by you and the student. Page two must be completed and signed by your family doctor. Return the complete form to the School Office. In addition, a current immunization record must be on file with the School.
All 7th Grade Students and New Students Grades 8 – 12 must provide the school with a DH-680 immunization form by the start of classes. This form is only available at your physician's office. The DH-680 form must show your student has had their adult Tetanus booster.
We recommend you visit your regular physician or a walk-in clinic.
