Junior Mountain Trip Forms
THESE FORMS ARE REQUIRED FOR ALL JUNIORS ATTENDING THE JUNIOR MOUNTAIN TRIP. All forms necessary for the 2010 Junior Mountain trip can be accessed at http://gutenberg.tampaprep.org/pisgahforms. Parents must complete the forms online, PRINT them out, sign them and send them to the school. THESE FORMS DO NOT SUBMIT AUTOMATICALLY. YOU MUST PRINT AND SIGN THE FORMS. All forms must be returned to Jean Mason by June 6, 2010.

ADDITIONAL INFORMATION REGARDING REQUIRED SCHOOL FORMS
2010-11 Absentee Form
INFORMATION UPDATE FORMThe School's Information Update Form should be completed and returned as soon as possible. Information received after July 30, 2010 will not be included in this year's directory. The information you write on the Information Update Form is the address and phone information that will be printed in the School Directory.
CONSENT AND AUTHORIZATION FORMThe School's Consent and Authorization Form must be read and signed by you and your student. The Consent and Authorization form must be returned to the School by July 30, 2010.BIRTH CERTIFICATEUnder the new guidelines of the Florida Council of Independent Schools (FCIS), we must have a birth certificate on file for each student to keep our accreditation. Please help us by sending in a copy of your student(s) birth certificate. If you have already submitted an official birth certificate, you need not submit a new one. Feel free to check with Melanie Rodriguez if you are unsure as to whether we already have a birth certificate for your child.
PHYSICAL EXAM Every student is required to have a yearly physical examination. Each physical is good for one calendar year. So that our records will be consistent, use the FHSAA physical examination form above. Page one must be completed and signed by you and the student. Page two must be completed and signed by your family doctor. Return the complete form to the School Office. In addition, a current immunization record must be on file with the School.
All 7th Grade Students and New Students Grades 8 – 12 must provide the school with a DH-680 immunization form by the start of classes. This form is only available at your physician's office. The DH-680 form must show your student has had their adult Tetanus booster.
